The District of Columbia Higher Education Licensure Commission serves as repository for transcripts and records of Capital University of Integrative Medicine's students and graduates.  Transcripts may be requested from the Commission.**

Special Note

Complete the electronic Transcript Request Form; providing the following information: name of institution, your valid email address, DOB, SSN, copy of valid photo ID and name and address of recipient(s). Transcripts are $10 each. Once the transcript is available, you will receive an email with payment instructions. Please allow 30 days for processing of transcript requests. For processing status inquiries please call: 202-481-3951.

 

**Only transcripts and related materials for students in good financial standing are provided.  CUIM never provided graduates with personal, academic, or clinical references.  Similarly, the DC Licensure Commission will provide only trancripts and related academic information as appropriate.  Official copies sent from the Commission will be identical to final CUIM transcripts, and will also bear the seal of the DC Education Licensure Commission.
